Generating robust evidence in CMHDs is limited by challenges in capturing meaningful change over time, including limited sensitivity of standard measures such as PHQ-9 and GAD-7, variation in how symptoms change over time that complicates phenotyping, and barriers to consistent data capture driven by stigma, privacy concerns, and fragmented care pathways. As a result, symptom and functional change are often difficult to reliably detect and interpret across research and care settings.
